~ Patient Referral Program ~

Great patients are the mainstay of Family Care Dentistry, and we rely heavily on word of mouth for growth. As a token of our appreciation for your referrals to our practice, we are pleased to announce our patient referral program:

• Receive a $50 or $20 gift certificate, good for any dental treatment in our office, for each patient (depending on age) you refer who has an exam, x-rays, and cleaning.
• New patients are asked upon arrival whom we can thank for referring them. When your name is given, your $50 or $20 gift certificate will be mailed directly to you.
• You receive a $50 gift certificate for patients referred who are 21 years old or over and a $20 gift certificate for patients who are under 21 years of age.
• Gift certificates are good toward dental treatment and may not be redeemed for cash, to pay existing balances, or used towards products.
